Cervical cancer is one of the most preventable cancers, yet it still affects many women and families across the world. This guide explains what cervical cancer is, what causes it, what symptoms deserve attention, how diagnosis and staging work, and how treatment is changing.

What is cervical cancer?

Cervical cancer is a cancer that starts in the cervix, the lower part of the uterus that opens into the vagina. In most cases, it develops slowly over years after a persistent infection with a high-risk type of human papillomavirus (HPV) causes abnormal cell changes that can eventually become cancer.

Most abnormal changes do not become cancer, and many HPV infections clear on their own. Trouble starts when a high-risk HPV infection stays in the cervix for a long time and gradually drives abnormal cell growth.

The two main types are squamous cell carcinoma and adenocarcinoma. The key point for patients is that cervical cancer often follows a long pathway, which is why vaccination, screening, and early treatment matter so much.

How common is cervical cancer?

Cervical cancer is the fourth most common cancer in women worldwide. In 2022, about 660,000 women were diagnosed and about 350,000 died, with the heaviest burden in low- and middle-income countries where access to vaccination, screening, and treatment is often limited.

In the United States, estimates for 2025 suggest about 13,360 new cases and 4,320 deaths. Countries with strong vaccination and screening programs have seen major declines, while limited-access settings still carry most deaths.

What causes cervical cancer?

Cervical cancer is caused in nearly all cases by persistent infection with a high-risk type of HPV. HPV is common, and most infections clear naturally, but persistent infection can lead to cell changes that become cancer over time.

HPV 16 and HPV 18 account for most cases worldwide, though other high-risk types also matter. Infection alone does not mean cancer will develop.

Factors that increase progression risk include smoking, immune suppression, HIV infection, long-term lack of screening, and delayed treatment of precancerous lesions.

What are the symptoms of cervical cancer?

Early cervical cancer often causes no symptoms. When symptoms appear, common warning signs include unusual vaginal bleeding, abnormal discharge, pelvic pain, or pain during sex.

  2. Bleeding between periods

  3. Bleeding after menopause

  4. Periods that become heavier or last longer than usual

  5. Watery, bloody, or foul-smelling vaginal discharge

  6. Pelvic pain

  7. Pain during sex

In more advanced disease, symptoms may include back pain, leg swelling, weight loss, fatigue, urinary issues, or bowel symptoms. Persistent bleeding or discharge should always be evaluated.

How is cervical cancer diagnosed?

Cervical cancer is diagnosed by examining the cervix, performing biopsy, and using imaging to assess tumor size and spread. Many cancers are first suspected after abnormal screening, but diagnosis is confirmed only after tissue testing.

Evaluation may include colposcopy with biopsy, followed by MRI, CT, or PET-CT for staging. Doctors then assign FIGO stage to guide treatment.

Screening and diagnosis are different: screening looks for warning changes, while diagnosis confirms whether invasive cancer is present.

How is cervical cancer treated?

Treatment depends on stage, tumor size, lymph node status, fertility goals, and overall health. Main options include surgery, radiation therapy, chemotherapy, and newer systemic therapies in selected settings.

Surgery

Surgery is often used in very early and some early-stage disease. Depending on stage, options include conization, simple hysterectomy, radical hysterectomy, and in selected patients, fertility-sparing surgery such as radical trachelectomy.

Potential effects include pain, bleeding, bladder or bowel changes, sexual side effects, and lymphedema if nodes are removed.

Radiation therapy

Radiation is central in many locally advanced cases. Standard treatment often includes external beam radiation plus brachytherapy. Brachytherapy is often a key curative component rather than an optional step.

Side effects can include fatigue, bowel/bladder irritation, vaginal dryness or narrowing, and early menopause in some cases.

Chemotherapy

Chemotherapy is commonly combined with radiation in locally advanced disease and used in systemic regimens for recurrent/metastatic disease. Cisplatin-based chemoradiation has long been a standard approach where radiation is indicated.

Side effects may include nausea, fatigue, low blood counts, neuropathy, hair loss, and infection risk.

New evidence (INTERLACE trial, 2024) showed improved outcomes with short induction chemotherapy before standard chemoradiation in locally advanced cervical cancer.

Immunotherapy and other advanced treatments

Newer options include immune checkpoint inhibitors (such as pembrolizumab), antiangiogenic therapy (bevacizumab), and antibody-drug conjugates (such as tisotumab vedotin in later-line settings).

KEYNOTE-A18 data support pembrolizumab with chemoradiotherapy in high-risk locally advanced disease where appropriate and available.

What is the outlook for people with cervical cancer?

Outlook depends on stage at diagnosis, nodal status, tumor biology, response to treatment, and access to specialized care. Earlier-stage diagnosis generally has better long-term control and cure potential.

Population statistics are useful context but cannot predict an individual outcome with certainty.

Living with cervical cancer: Practical tips for patients and families

Living with cervical cancer often means managing treatment effects, emotional stress, and daily life disruptions at the same time.

  1. Keep a simple record of bleeding, pain, bowel/urinary symptoms, fatigue, and treatment side effects

  2. Tell your team early if eating, sleep, mobility, or mood is worsening

  3. Ask about fertility, menopause, sexual health, and vaginal care before treatment starts

  4. Request support for pain, anxiety, and low mood - supportive care is part of cancer care

  5. If you smoke, ask for structured help to stop

Follow-up after treatment should include recurrence monitoring plus support for bowel, bladder, sexual, and mental health concerns.

When should you talk to a cancer specialist about cervical cancer?

Speak with a specialist promptly if you have biopsy-confirmed cervical cancer, suspicious cervical findings, unexplained abnormal bleeding, or an abnormal screening result that needs expert follow-up.

Early specialist input can influence staging, fertility decisions, treatment sequencing, and access to newer therapies.

Frequently Asked Questions about Cervical Cancer

Can cervical cancer be prevented?

In many cases, yes. HPV vaccination, regular screening, and treatment of precancerous lesions can prevent most cervical cancer cases.

Does an HPV infection mean I have cervical cancer?

No. HPV is common, and most infections clear naturally. Cancer typically develops only when high-risk HPV persists over time and drives cell changes.

Can cervical cancer still happen if I had screening before?

Yes. Screening lowers risk and often detects disease earlier, but no screening program is perfect. Persistent or new symptoms should still be evaluated.

Is cervical cancer curable?

Many cases are curable, especially when found early. Even when cure is not possible, treatment can still control disease and improve quality of life.

Will treatment affect fertility or menopause?

It can. Some early cancers may allow fertility-sparing surgery, while radiation and some surgeries can affect fertility and trigger menopause.
